Advance directives are legal documents that provide instructions for medical care when you cannot communicate your own wishes. The two most common types are: Living will: Specifies what actions should be taken for your health if you are unable to make decisions for yourself due to illness or incapacity, and Durable power of attorney for health care: Designates someone to make medical decisions on your behalf. The process of making care decision known, prior to the need for a life-sustaining decision, is referred to as, “Advanced CARE Planning.” The Bourg Society has included an example of a Living Will and Power of Attorney for Decisions, and a list of relative terminology, to allow you, your family, and caregivers, to become familiar with the process.
